You’d be surprised of how many of your nursing needs can be met in the comfort of your own home. Help on Wheels (HOW) nurse works together with your doctor, hospital and other allied health staff to ensure you are well taken care of and saving you trips to the hospital and doctor, giving you more time for the things you truly enjoy.

This service provides specific Nursing Care including:

  • Wound care
  • Continence care, including catheters and product selection
  • Bowel care
  • Medication management, including injections
  • Chronic disease management such as diabetes
  • Post-hospitalisation care
  • Dementia care
  • Rehabilitation
National Disability Insurance Scheme (NDIS)

Help on Wheels (HOW) is a registered National Disability Insurance Scheme (NDIS) provider dedicated to supporting people with disabilities. We focus on your abilities and strengths, rather than things you aren’t able to do. Using these as a base, our flexible services will assist you to participate in the community and live as you wish — safely, independently, and guided by your individual needs and preferences. We find ways to help you maintain or increase your independence and achieve your health and wellbeing goals.

Our approved NDIS services include:

  1. Assist with travel/transport
  2. Community Nursing
  3. Assist with personal activities
  4. Assist with high intensity activities

Assist with travel/transport

Getting to and from school, work, or travelling around your community can be difficult if you’re not able to drive or use public transport because of your disability. If you are an NDIS participant and have been approved for the support category ‘transport’ in your NDIS plan, you can access assistance with travel/transport arrangements. We provide you with safe, convenient transport, scheduled in advance to help you get to:

  • School or another educational facility
  • Work
  • Your doctor or other medical appointments
  • Your local shopping centre
  • Around your community

Help on Wheels (HOW) staff member will drive you to your appointment or another place in the community and take you back home when you’re done. Our transport services are booked on an ongoing basis, such as a weekly trip to the shops or regular medical appointments.

This means HOW can connect you with the wider community, support you to learn new skills, and assist you with day-to-day activities. We can also help you to understand how the NDIS works, and to make informed choices.

Community Nursing:

We provide community nursing to participants who are eligible for NDIS and Department of Veterans’ Affairs services. We offer a range of home nursing and personal care services to help you maintain your health or maximise your independence. If you are an NDIS participant and have been approved for the support category ‘Community Nursing’ in your NDIS plan, you can access the supports below.

  • Wound care – wound assessment, changing and maintenance of dressings
  • Medication management – such as administration of medication, insulin for example
  • Nursing assessments – a comprehensive review of your current health needs. If we identify that you need the assistance of other allied health professionals or a doctor we contact them on your behalf.The personal care services we offer through community nursing include:
  • Bathing and showering
  • Dressing (including putting on compression stockings)
  • Medication assistance – assistance with removing tablets from bottles or blister packs.

Assist with Personal Activities

We provide assistance with a range of everyday activities in the comfort of your home, supporting you to learn the skills needed to be as independent as possible. We are also able to provide these supports to people with complex needs. Personal activities can include:

  • Showering or bathing
  • Dressing
  • Grooming
  • Personal hygiene
  • Going to the toilet
  • Getting in and out of bed
  • Help with eating
  • Moving around the house

Assist with High Intensity Activities

We can assist with and/or supervise personal tasks of daily life to enable participants to live autonomously and comfortably. HOW has highly qualified and trained employees who are passionate and eager to assist participants who require Assist with high intensity activities.
Some of our In-home supports include but not limited to:

  • Assistance with showering, dressing and grooming.
  • Support with shopping, meal preparation and cooking.
  • Assistance with mobility and exercising to maintain a healthy lifestyle.
  • Assisting participants overnight.
  • Basic domestic duties.
  • Assistance with meal preparations.
  • Active care overnight (carer would be awake).
  • Assisting and caring indwelling catheter (IDC)
  • Assisting and caring suprapubic catheter (SPC)
  • Assisting and caring colostomy bag
  • Assisting in caring with tracheostomy
  • Assisting participants to access other programs (swimming club, sports, or their local RSL club).
  • Assisting participants shopping.
  • Assisting participants to attend appointments.
  • Support participants to access recreational activities such as a concert or gallery.
  • Assisting participants to attend community events.

Who We Support

We provide services to support children, young people and adults with special needs; intellectual disabilities (including developmental delay, and down syndrome); physical disabilities (including cerebral palsy, epilepsy, and brain injury); and sensory disabilities (including visual impairment, hearing loss, and autism). We also support parents, carers and families caring for a child or adult with disabilities.

Cost of Supports

The NDIA publishes a price list, which sets out the cost of each support. This is the cost of services, no matter which provider you choose. The price list is valid from 1 July each year and lasts for 12 months.
In some cases, the cost of a support may not be fixed, and we can provide you with a quote that the NDIA will approve.
There are no out-of-pocket expenses for accessing the supports in your plan.

Domestic Care

Living in a clean, tidy and safe home improves your quality of life and contributes to your wellbeing. If you find it difficult to keep up with your everyday household duties, Help on Wheels (HOW) Care Workers will lighten the load through hands on support with duties such as:

  • Cleaning
  • Washing
  • Ironing
  • Dusting
  • Sweeping and vacuuming floors

  • Light meal preparation

Social Support

Connecting with your friends, family and relatives have a profound impact on your physical, mental and emotional health. Social support, from taking you to Doctors appointments to going to the Opera or just sitting with you at the club playing pokies or bingo. Why be lonely or bored, let us assist you do as you want! Our social support services include:

  • Accompanied shopping, bill paying, banking
  • Transport to social activities such as your local club, your local markets and your church
  • Visit to friends or family
  • Doctor’s appointment
Transport

Respite

Respite care can support you and your carer with a break for a short period of time. This gives carers the chance to get to everyday activities or go on a planned break. Respite care may be given informally by family, friends or neighbours, or by formal respite care services. Help on Wheels (HOW) care worker allows the carer to focus on their health with the security that their loved one is taken care of by someone

  • At home: home respite can provide a few hours break on a regular basis.
  • Overnight respite may be provided in the home of a host family.
Home Maintenance

Help on Wheels (HOW) home maintenance service provides a modification, maintenance, adjustment and advice for frail older people, younger people with disability or functional limitations and their carers.

Our Home Maintenance services include:

  •  Replacing tap washers
  • Installing smoke alarms, or changing the batteries
  • Planing doors which have become stuck
  • Replacing hinges or knob
  • Lawns, changing light bulbs,
  • Pool care,
  • Flyscreens, security doors
  • Moving furniture, rubbish removal, yard clean ups, blocked drains and plumming, gutter cleaning
  • Specialising in modifications for disabilities and after hospital treatment eg hand rails, ramps
  • Mowing the lawn and watering your garden
  • Clearing pathways to and from your door
  • Undertaking minor repairs to steps, floors and railings
  • Installing locks and sensor lights
  • Managing trusted contractors to clean your gutters and windows
