How It Works

Planned with you, delivered with clinical oversight

Some conditions change over time, and support that worked last year may not fit today. Our registered nurses assess where things are now, write the plan with you, and adjust it as your health, energy and routines shift.

We match workers who understand your condition, keep your GP and treating team informed, and stay in touch with the people closest to you so nothing gets missed.

    Nurse assessment, product guidance and written reporting for continence needs — we support people of all ages.

  • Spinal cord injury support

    Support after spinal cord injury covering transfers, skin integrity, bladder and bowel routines and getting out into the community.

  • Muscular dystrophy support

    Support that adapts as strength, mobility and fatigue change, with attention to positioning, breathing and equipment.

  • Multiple sclerosis support

    Flexible support through relapses, fatigue and mobility changes, with more help available when you need it.

  • Parkinson's support

    Support arranged around medication windows, movement changes and the routines that keep your day steady.

  • Diabetes and insulin management

    Support with blood glucose monitoring, insulin administration, foot and skin checks, and meal routines.

  • Recovery after a hospital stay

    Short-term support to settle back in at home — medications, wound care, appointments and building confidence again.

All supports are delivered as a registered NDIS provider, in line with the NDIS Code of Conduct and NDIS Practice Standards overseen by the NDIS Quality and Safeguards Commission.
