Specialist Support
Specialist support for changing health needs
Condition-focused support planned with registered nurse oversight — for people living with dementia, neurological conditions, spinal cord injury, diabetes and more.
- Registered NDIS provider
- Registered nurse oversight
- Across Greater Sydney
How It Works
Planned with you, delivered with clinical oversight
Some conditions change over time, and support that worked last year may not fit today. Our registered nurses assess where things are now, write the plan with you, and adjust it as your health, energy and routines shift.
We match workers who understand your condition, keep your GP and treating team informed, and stay in touch with the people closest to you so nothing gets missed.
How we work with you
- An unhurried first conversation about what you need and what your plan can fund
- A written support plan, with workers matched to your routine and preferences
- Regular reviews with nursing oversight, so your support changes when your needs do
