What Community Access & Social Activities involves
Connecting with friends, family and your community matters to your physical, mental and emotional health.
Our support workers can accompany you to appointments, outings and activities — from a trip to the local markets to an afternoon at your club. You choose what you would like to do.

Social support we provide
Getting out and about looks different for everyone. It might be a regular outing you look forward to, or simply having someone alongside you so an errand feels manageable.

How we make outings work
We plan outings around you rather than around a roster, taking account of your energy, your equipment and the times of day that suit you best.

Getting Started
How we work with you
Support only works when it is built around the person receiving it. Before anything begins, one of our registered nurses sits down with you — and anyone else you want in the room — to understand your goals, your routine and how you like things done.
From there we write it down in plain language, introduce you to the workers who suit you, and keep checking that the arrangement still fits. Nothing is locked in, and you can change your mind as you go.
- An unhurried first conversation about what you need and what your plan can fund
- A written support plan, with workers matched to your routine and preferences
- Regular reviews with nursing oversight, so your support changes when your needs do
